Rehabilitation Services

Our rehabilitation program meets the needs of those who are recovering from a hospital stay or illness. Physical, occupational, speech and IV therapy programs are designed to restore each patient to his or her highest level of functioning and independence. Each patient’s course of treatment is reviewed daily in a multi–discipline management meeting. Goals are related to a positive patient outcome for a safe and timely return home. The Activity staff is dedicated to creating programs targeted toward all levels of participation; offering activities designed to meet the social, recreational, physical and spiritual needs.

Jeff R. Sydlowski

Occupational Therapist

Graduate Degree in Bachelors of Science in Occupational Therapy. Jeff has worked in hospitals and skilled nursing facilities for a total of 16 years.

Stephanie Graves

Therapy Director

Graduate with an Associates Degree in Occupational Therapy. Stephanie has two years experience in acute hospitals and twelve years experience in skilled nursing facilities.

David Lonergan

Physical Therapist

Graduate Degree in Bachelor of Science in Human Biology and Doctorate Degree in Physical Therapy. David has been working in hospitals since 1993 and skilled nursing facilities since 2001. David is a member of the Physical Therapy Association.
